Physician Referrals
Most insurances no longer require a referral for patients to attend therapy.
If needed, please include frequency and duration on the referral form. If we discover one is needed after care is started, we will send one to your physician for completion.
Medicare requires a physician of record to sign our plan of care (POC) for you to continue care with us after your initial session.

We are working diligently with our contracts and credentialing team to complete this process for all major plans, and waiting on the insurance companies to complete their work.
